The authority for collecting this information is contained in the Consolidated Farm and Rural Development Act (7 U.S.C. 1921 et seq), Title V of the Housing Act of 1949 as amended (42 U.S.C. 1471 et seq), 7 C.F.R. Section 3560, subsection 55 & 154, and Pub. L. 117-2 (American Rescue Plan Act of 2021). The principal purpose for which the information is intended to be used is to process inquiries from existing Single-Family Housing Direct Loan borrowers for financial assistance. Providing this information is voluntary, however, failure to provide the requested information, specifically the full name, telephone number, email address, and last four digits of the Single-Family Housing Direct Loan account number, may result in the inability to process the request for financial assistance. The information collected on this form will be included in a Privacy Act System of Records known as USDA RD-1, titled “Current or Prospective Producers or Landowners, Applicants, Borrowers, Grantees, Tenants, and Other Participants in RD Programs” and will be subject to the routines uses published in the System of Records Notice for USDA RD-1 (please see https://www.ocio.usda.gov/policy-directives-records-forms/records-management/system-records) including:

Disclosure of names, home addresses, and financial information for selected borrowers to financial consultants, advisors, lending institutions, packagers, agents, and private or commercial credit sources, when Rural Development determines such referral is appropriate to encourage the borrower to refinance their Rural Development indebtedness as required by Title V of the Housing Act of 1949, as amended (42 U.S.C 1471), or to assist the borrower in the sale of property.
